The Middletown Community Based Outpatient Clinic (CBOC), a support facility of the Providence VA Medical Center, was opened on November 13, 2000 and provides comprehensive primary healthcare services to veterans in Newport, Washington, and Bristol counties. The facility offers patients primary care, mental health, and pharmacy services. It is located at One Corporate Place in Middleton, Rhode Island and cancer be reached by phone at (401) 847-6239. The Middletown CBOC’s hours of operation are Monday through Friday, from 8:00 a.m. to 4 p.m. As one of five support clinics of the Providence VA Medical Center, the Middletown CBOC offers its services to local patients in a smaller, easier-to-access location. Furthermore, patients at the Middletown CBOC have primary care providers to help coordinate patient healthcare needs and ensure easy access to any of the Providence VA Medical Center’s 32 subspecialty clinics. Furthermore, patients can receive referral to the main VA center for a wider array of service, including inpatient care. For information on enrolling for service in this clinic, patients can call (401) 457-3300 or the Telephone Assistant Point (TAP) Line at (401) 457-3336. Before patients visit the Middleton CBOC, or any VA healthcare facility, they must first determine their eligibility by applying for healthcare benefits. Following confirmation of eligibility, patients can schedule an appointment with the VA medical center of their choice, as walk-in appointments are not given. In order to serve the maximum number of veterans on a daily basis, patients are encouraged to notify the facility as soon as possible if an appointment cannot be kept. This enables for the quick rescheduling of the appointment, as well as giving the clinic the ability to schedule another patient in the vacated time slot.
